Please provide additional description of the work which you believe may be responsible for any damage reported, along with any specific question(s) you may have. To the extent you have it, some relevant information might be:
  • approximate dates of work or date of damage, if known,
  • whether other homes in the area are damaged,
  • the type and amount of documentation you have of both the damage and all the construction procedures used
  • construction activities and equipment considered likely to have been at fault,
  • the foundation type of your home (concrete slab-on-grade, rubble, concrete block, etc.),
  • the result of any damage claim you may have filed
  • plus any others which you consider relevant,
  • an explanation of any selection of "Other" in the drop-down menus above.
